The certificate of nomination at the county primary election for a candidate without opposition shall be in the following form:

CERTIFICATE OF NOMINATION
THIS IS TO CERTIFY THAT
having no opposition at the primary election is hereby nominated by the ____________ Party for the office of _________________________.
