The capital outlay ballot must be white and in the following form:

OFFICIAL SCHOOL CAPITAL OUTLAY CERTIFICATES ELECTION BALLOT

SCHOOL DISTRICT NO.

SOUTH DAKOTA

(Election Date)

The following decision was passed by the school board and submitted to the voters. The decision does not become effective unless approved by sixty percent of the vote.

(HERE LIST THE AMOUNT AND TERMS OF THE INSTALLMENT PURCHASE CONTRACT OR LEASE-PURCHASE, OR ISSUANCE OF CAPITAL OUTLAY CERTIFICATES)

To vote use a cross (X) or check mark (TICK) in the square in front of "Yes" or "No".

[] Yes A vote "Yes" will approve the board's capital outlay certificates decision.

[] No A vote "No" will not approve the board's capital outlay certificates decision.
