The special district formation ballot must be white and must be in the following form:

OFFICIAL BALLOT

FORMATION OF ____________ DISTRICT

(Election Date)

The following question was proposed by (petition) (the county commission) for submission to the voters. This district formation will not become effective unless approved by (majority vote) (sixty percent of votes cast).

Shall the ____________ District be formed?

To vote use a cross (X) or check mark (T) in the square in front of "Yes" or "No".

[] Yes A vote "Yes" will form the ___________ District.

[] No A vote "No" will not form the ___________ District.
