Each employer, when requested by the Department of Labor and Regulation or upon becoming a liable employer in this state, shall file with the Department of Labor and Regulation a report to determine liability. The report shall contain, but not be limited to, the following:

(1) Employer's name, address, and type of organization;

(2) Work locations and dates of employment; and

(3) Employees' earnings.
