If the individual has a developmental disability, developmental delay, traumatic brain injury, or chronic medical condition, a copy of a document containing the diagnosis and name of the physician, psychiatrist, or psychologist issuing the diagnosis must be submitted with the application.

If the individual is a child with a serious emotional disturbance or if the individual is an adult with a severe and persistent mental illness, a Mental Health Summary Evaluation form as provided by the division must be submitted with the application.

If a post-adoptive family is seeking services, a copy of the court order of adoption must be submitted with the completed application.
