A medical exemption pursuant to SDCL 13-28-7.1 requires the signature of a physician licensed pursuant to SDCL chapter 36-4. The signature shall be on the South Dakota Department of Health - Certificate of Immunization form.

A religious exemption pursuant to SDCL 13-28-7.1 requires the signature of the parent or guardian on the South Dakota Department of Health - Certificate of Immunization form or a signed statement by the parent or guardian stating that child is adherent to a religious doctrine whose teachings are opposed to immunization.

The school or early childhood program is required to collect the exemption documents and submit to the department no later than November 1 of the current school year or within 45 days of transferring into the school district.
