Before a J-1 physician may be transferred to a different practice location, the employing facility shall complete the "J-1 Visa Physician Transfer Application Form" and submit the completed form to the department for review. If the transfer application materials support the requested transfer, a letter of concurrence signed by the Secretary shall be sent to the current employing facility. A copy of the physician's notification to USCIS and the U.S. Department of State shall be submitted to the department following approval of the transfer.
