Commencing one year after the J-1 physician begins employment with the employing facility and every year thereafter for a period of three years, a representative of the employing facility and the J-1 physician shall certify the employment status of the J-1 physician. The department shall send a form to the employing facility and J-1 physician which includes the following:

(1) Any changes in the J-1 physician's practice location;

(2) Any changes in the J-1 physician's employment status; and

(3) Dated signatures of both the J-1 physician and either the chief executive officer of the employing facility or a representative of the chief executive officer.
