The regionally accredited institution of higher education offering the educator preparation program attended by the student teacher shall provide permission and sign off on the student teacher's assignment, with acknowledgement that the student teacher candidate has successfully completed the requirements as determined by the regionally accredited educator preparation program including, but not limited to:

1. Successful completion of the state-designated pedagogy test;

2. Successful completion of the state-designated content test(s) in the area(s) assigned;

3. A minimum level of proficiency of a 3.0 grade point average for content and pedagogy coursework;

4. Successful completion of coursework expected of the student teacher candidate to qualify for student teaching experience at the time of application; and

5. Registration for the final coursework necessary for program completion, with such coursework reflected on an official transcript.
