When facilitating a transport, the certified professional midwife shall notify the hospital when the transport is initiated, accompany the client to the hospital if feasible, or communicate by telephone with the hospital if the certified professional midwife is unable to be present. The certified professional midwife shall also ensure that the transfer of care is accompanied by the clients medical record and the a transport form prescribed by the board.
