An applicant may seek relicensure if the applicant has been licensed in this state and either failed to timely renew or is seeking to return to active clinical practice. The following must be submitted at the time of reapplication:

(1) A completed application and payment of fee;

(2) A current CPM certification from NARM;

(3) Satisfactory explanation for such failure to renew; and

(4) Evidence of employment status during the preceding six years as described in SDCL 36-9C-16.
