Section 20:86:02:01 - Qualifications for licensure

No person may be licensed to practice as a certified professional midwife in this state unless the person has completed the requirements set forth in SDCL 36-9C-4. In addition, each applicant shall ensure that the board receives all documentation necessary to prove to the Boards satisfaction that the applicant meets all the requirements for licensure. Every applicant shall provide:

(1) Completed application and fee;

(2) Evidence they have not been convicted of a crime which in the judgment of the board renders the person unfit to practice midwifery;

(3) Fingerprints and other information necessary for a criminal history check; and

(4) Applicant may be required to appear for a personal interview with the Board if deemed appropriate by the board.
