When the subscriber has paid bills for service for 12 consecutive months without having service disconnected for nonpayment and without receiving three or more disconnection notices, the telecommunications company shall promptly and automatically refund the deposit plus accrued interest to the customer in the form of cash or credit to the subscriber's bill. The subscriber shall choose the form of the refund.
