A petition for a contested case hearing shall request a hearing before the commission and shall contain a statement of the petitioner's interest in the case, a statement of the relief and decision requested of the commission, a statement alleging relevant facts and issues known to the petitioner upon which be bases his request of the commission, and the signature of the petitioner or any other person who will represent the petitioner before the commission. The signature on a petition, or any other pleading, shall constitute a certificate by the signer that he has read the petition or pleading, that to the best of his knowledge, information, and belief there are good grounds to support the claim, and that it is not filed for purposes of delay.
