The application shall be submitted in duplicate, identifying all parties known to the applicant by name or by class to be affected by the outcome of such ruling. The application shall also clearly identify the statute, rule or order sought to be interpreted and concisely state the factual considerations which necessitate the ruling. The applicant shall sign and verify the same to be true to the best of his knowledge and belief. The application and copy shall be filed with the executive secretary who shall immediately transmit copies thereof to the members of the commission.
