Section 61-107.3.III.E - Application Requirements for Waste Tire Collection Facility Permits

1. The application for a waste tire collection facility shall be on a form provided by the Department.
2. The application shall contain at a minimum the following:
a.
Proof of ownership of the property upon which the waste tires are
collected;
b. Site maps or other
documents detailing the location and design criteria requirements of Part
III.C;
c. An operational plan
outlining the operational requirements of Part III.D;
d. A closure plan which shall include at a
minimum the following:
(1) Schedule for
removal of all waste, waste tires, or processed tires and residuals;
and,
(2) Certification that all
waste or processed tires remaining on site will be transported to a permitted
processing or disposal facility and that closure shall be performed in
accordance with Part V of this regulation; and
e. Documentation that the applicant has
secured financial assurance in accordance with Part V of this regulation.
