| The Panel shall charge the following fees: | |
| A. Application: | $175 |
| B. Biennial Renewal of License: | $100 |
| C. Late Renewal Fee: | $50 up to thirty days |
| D. Reinstatement Fee: $50 in addition to the renewal and late renewal fees | |
| E. Miscellaneous: | |
| 1. Replacement of wall certificate: | $15 |
| 2. Duplicate pocket card: | $10 |
| 3. Name change and new pocket card: | $10 |
| 4. Fee for returned check: | $30 |
| 5. Licensure verification: | $5 |
| 6. Licensure list: | $10 |
