27.5.1 Documentation
A. In order to apply for a license an
applicant must submit the following on forms provided by the Department:
1. Completed application including but not
limited to name, address, date of birth, social security number, telephone
number and email address;
2.
Application fee as defined in Part 10-05-2 of this Title, the Rules and
Regulations Pertaining to the Fee Structure for Licensing, Laboratory and
Administrative Services Provided by the Department of Health. The fee is
non-refundable and non-returnable;
3. Verification of current board
certification as an International Board Certified Lactation Consultant
submitted directly to the Department by the International Board of Lactation
Consultant Examiners.
27.5.2 Licensure by Endorsement
A. In order to apply for a license by
endorsement an applicant must submit the following on forms provided by the
Department:
1. Completed application
including but not limited to name, address, date of birth, social security
number, telephone number and email address;
2. Application fee as defined in Part 10-05-2
of this Title, the Rules and Regulations Pertaining to the Fee Structure for
Licensing, Laboratory and Administrative Services Provided by the Department of
Health. The fee is non-refundable and non-returnable;
3. Verification of current board
certification as an International Board Certified Lactation Consultant
submitted directly to the Department by the International Board of Lactation
Consultant Examiners; and
4.
Verification that the applicant is licensed and in good standing from all
states where the applicant is currently licensed and, if applicable, previously
licensed as a lactation consultant.
