(a) A transporter shall submit to the Department's Central Office an annual report. The report shall be submitted by the end of March of each calendar year. The report shall be submitted on forms supplied by the Department.
(b) The annual report shall be based on the shipments of regulated medical or chemotherapeutic waste during the previous calendar year, and shall include the following:
(1) The name, location, telephone number and
permit identification number of each processing or disposal facility to which
the transporter delivered regulated medical or chemotherapeutic
waste.
(2) The weight or volume of
each type of regulated medical or chemotherapeutic waste transported.
(3) When more than one transporter is used to
transport a single shipment of regulated medical or chemotherapeutic waste from
the generator to the processing or disposal facility, only the first
transporter is required to submit information for that shipment on the annual
report.
