An applicant for CMA reinstatement must submit a fully completed application established by the Board.
(1) An applicant for reinstatement of a CMA certification must:
(a) Meet all the terms and
conditions of reinstatement imposed by the Board; and
(b) Take a Board-approved medication aide
training program; and
(c) Pass the
competency exam within one year of the date of training program completion.
There is no limit on the number of times an individual can take the exam within
the one year period.
(2) Reinstatement is required for certification that was voluntarily surrendered by the CMA or revoked by the Board.
