(1) "Hand off", means a transfer and acceptance of nursing services responsibility for a client achieved through effective communication. It is a synchronous process of passing client-specific information from one nurse to another for the purpose of ensuring continuity of services and the safety of the client.

(2) "Health Care Team" means those working with the client to achieve the client's identified outcomes. The composition of the health care team is appropriate to the context of care and must always include the client. The team is not limited to licensed health professionals.

(3) "Holistic Health Care" means an approach to diagnosis and treatment of clients, which considers the status of the whole person: physical, emotional, social, spiritual, and environmental.
