Section 839-014-0260 - Procedure for Filing Protest

(1) Any individual desiring to protest the issuance of an indorsement must file the protest in writing with the Bureau of Labor and Industries.
(2) The written protest must contain the following information:
(a) Name, address and
phone number of the individual filing the protest;
(b) Date of the protest;
(c) Name of indorsee or applicant against
whom the protest is being made;
(d)
A complete statement of the facts, circumstances and other reasons for the
protest. The statement may include alleged violations, approximate dates of
alleged violations, names of witnesses, if any, and any documents which support
the allegations; and
(e) The
signature of the individual making the protest.
