Section 833-075-0040 - Duplicate Licenses

(1) Duplicate licenses or certificates may be obtained by:
(a) Certifying, by signed statement, that the
current license or certificate has been lost or destroyed; or
(b) Requesting a duplicate for additional
place(s) of business;
(c) In
either case, payment of the required fee.
(2) Reproduction of a license or certificate by anyone other than Board staff for use as a license document or for display is prohibited.
