(1) All private duty nursing services require prior authorization.
(2) Private duty nursing visit:
(a) T1030 - Nursing care in the home by
registered nurse per diem;
(b)
T1031 - Nursing care in the home by licensed practical nurse per diem.
(3) Private duty nursing shift care:
(a) S9123 - Nursing care
in the home by registered nurse per hour -- 1 unit equals one hour;
(b) S9124 - Nursing care in the home by
licensed practical nurse per hour - 1 unit equals one hour.
