Section 331-910-0022 - Application Requirements for Electrology Temporary Trainee License

An individual applying for an electrology temporary trainee license must:

(1) Meet the requirements of OAR 331, division 30;

(2) Submit a completed application form prescribed by the Office, which must contain the information listed in OAR 331-030-0000 and be accompanied by payment of all required fees;

(3) Submit proof of being at least 18 years of age; acceptable forms of identification listed under OAR 331-030-0000; and

(4) Submit proof of having a high school diploma or equivalent education.
