(1) All levels of certification of any fire service professional will be considered lapsed if the individual has not been utilized as such for more than 12 consecutive months.
(2) A fire service professional whose certification has lapsed must apply for re-certification upon re-utilization as a fire service professional.
(a) The fire service professional must
complete a Department task book, task performance evaluation or approved
training.
(b) Upon successful
completion of the appropriate testing or evaluation, as verified by the
signature and recommendation of the agency head or designee, the fire service
professional whose certification has lapsed, may request reinstatement of
certification. The request must be made to the Department by submitting the
appropriate form.
