Section 160-100-0220 - Notification of Secretary of State of Lost or Stolen Journal

(1) A notary public whose notarial journal is lost, misplaced, destroyed or otherwise unusable may file a written notice, under oath or affirmation, with the Secretary of State.
(2) The notice shall set forth:
(a) A statement of whether the notarial
journal is lost, misplaced, destroyed or in some other manner made unusable;
(b) An explanation of how the
notarial journal became unusable.
(c) The date the notary public discovered
that the notarial journal was unusable.
(d) A statement that the notary public does
not possess the notarial journal and does not know who possesses it or where it
is located.
