Section 612:25-4-58 - Annual and Performance Evaluations

(a) The BEP will conduct an annual evaluation of each licensed manager at the end of each calendar year. Performance evaluations will be conducted when a manager applies for a facility or when a probationary period ends. All evaluations will be based on data collected from the manager's primary assigned facility and will consist of the previous twelve (12) working month's information. If a Licensed Manager does not have a primary facility, the satellite facility will be used (in accordance with BEP 612:25-4-53(c)(1-5). The Licensed Manager will be advised of the results of any evaluation in writing. The manager will be evaluated in the following areas:
(b) When a Licensed Manager applies for a facility a performance evaluation will be conducted to determine their eligibility to apply. To be eligible the manager must score at least 80 of the available 100 points. Any score above 80 will accrue to the benefit of the licensed manager in that selection process by adding it to their total score. If there is a second interview conducted, these points will not be added to the total points of the second interview.

1This emergency action expired without being superseded by a permanent action. Upon expiration of an emergency amendatory action, the last effective permanent text of the Section is reinstated. Therefore, on 7-15-00 (after the 7-14-00 expiration of the emergency action), the text of 612:25-4-58 reverted back to the permanent text that became effective on 7-1-97, as was last published in the 1999 OAC Supplement, and remained as such until amended by permanent action on 7-1-02.
