(a) All motor vehicle accidents and collisions, required to be reported by law, shall be reported by the investigating law enforcement agency on the "Official Oklahoma Traffic Collision Report". The latest version of the "Official Oklahoma Traffic Collision Report" is available from the Department of Public Safety on its website at https://oklahoma.gov/dps/forms/handwritten-collision-report-form.html.

(b) Official Oklahoma Traffic Collision Reports may be submitted by the investigating law enforcement agency to the Service Oklahoma, Records Management Division, PO Box 11415, Oklahoma City, OK 73136.
