Section 390:45-1-6 - Application procedure

(a) Any person who wishes to apply for a permit under the Act, may do so in writing to CLEET, 2401 Egypt Road, Ada, Oklahoma 74820-0669.

(b) The completed application form shall constitute a sworn affidavit signed by the applicant attesting to eligibility requirements and notarized.

(c) All applicants must submit a notarized statement from the agency head of the retiree's agency verifying the status of the applicant as a retired peace officer of that jurisdiction.

(d) Retired reserve officers must submit the same documentation indicated in (c) above, and meet the eligibility requirements provided in 390:45-1-5.

(e) All information on the application or any related document must be complete and accurate to the best of the applicant's knowledge.
