(a) All submissions of evidence to an OSBI Laboratory shall be accompanied with an RFLE. The only exceptions are as follows:
(1) Mailed blood alcohol kits-A
photocopy of the kit form shall replace the RFLE;
(2) Convicted felon DNA kits DNA offender
samples
(b) The requesting/submitting officer shall completely and legibly fill out the RFLE.
(1) All evidence submitted shall be itemized
on the RFLE or an attachment to the RFLE.
(2) A statement of facts or incident report
and submitting agency's case number may be attached to the RFLE. This
information may be forwarded to the OSBI laboratory at a later date if
requested by an OSBI Laboratory Analyst.
(3) The RFLE should also clarify the types of
examinations requested and any special needs of the requesting/submitting
agency.
(4) Prior to submitting an
RFLE with evidence, the submitting officer shall:
(A) Unload all firearms submitted and
document the unloading on both the RFLE and the container holding the
firearm.
(B) Label and package all
known carcinogens, mutagens, highly toxic substances, volatile or foul smelling
chemicals or compounds in a manner to assure the safety and security of OSBI
Laboratory personnel.
(C) Properly
and safely package and label knives and other sharp objects and biological
samples using hazard labels and protective containers.
(5) A separate RFLE must be completed for
each case, and for each time additional evidence is submitted for an existing
laboratory case.
