Section 340:2-5-114 - Hearing request

(a) A hearing request must be in writing and signed by the appealing party.
(b) To be considered timely, a hearing request must be received by the Oklahoma Department of Human Services within:
(1) 30 - calendar days of receipt of the
notice of denial or revocation;
(2)
10-calendar days of receipt of an Emergency Order; or
(3) 30-calendar days of receipt of the notice
of recording on the Child Care Restricted Registry.
