(a) Each application shall include the following documents:
(1) Application form;
(2) Official documentation showing the
applicant meets the requirements listed in OAC 310:395-7-2; and
(3) Fee(s).
(b) The application form requires the following:
(1) Identifying
information;
(2) Resume with
relevant work history;
(3)
Possession of other credentials;
(4) Previous misconduct or disciplinary
actions;
(5) Declaration of
previous court judgements against the applicant related to midwifery care; and
(6) Other information that may be
required by the Department.
