Section 260:135-3-71 - Corrections

(a) If information on file is incorrect and the information to be corrected has not been authorized to the licensed operator, the applicant must appear before a Driver License Examiner with proper documentation.

(b) The licensed operator has the authority to make changes in height, weight, address, and last name as a result of marriage or divorce. To change the last name as a result of marriage, documentary evidence is necessary. To change the last name as a result of divorce, the licensed operator shall read the divorce decree and place the name on the driver license precisely as stated in the court order without any changes. For detailed instructions, see OAC 260:135-5-16. A copy of the divorce decree need not accompany the application. Corrections to a misspelled name shall be presented to an Examiner to verify and correct.

(c) All documents used for updates or corrections to the driver license or identification card file, must be the original or a certified copy. A petition or an application shall not be acceptable.
