License fees shall be established by the board in amounts not to exceed the maximum allowable under Chapter 4758. of the Revised Code. Fees are subject to change by action of the board, the controlling board, and/or the general assembly.

(A) Fees shall be paid by , credit card unless otherwise specified by the Board's executive director on a case-by-case basis for extenuating circumstances.

(B) Fees shall be submitted at the time of the initial application.

(C) Fees are non-refundable.

CEU Program Sponsor Application

$ 20.00 per application, $5.00 for each CEU hour after the first hour/per day training. $60.00 Multi-sessions' trainings. $350.00 for CDCA course approval good for two years. A twenty-dollar late fee shall be charged for any program sponsor application not received by the board at least thirty days before the training.

CEU Provider Status Application

$400.00 per one year or $700.00 per two years
