(A) The course instructor shall issue a "Mature Driver Course Completion Certificate" certifying successful completion of the course by each participant. The "Mature Driver Course Completion Certificate" shall be furnished by the director of the department of public safety.

(B) The "Mature Driver Course Completion Certificate" shall include the name of the student, the name of the course instructor, the date the course was completed, and shall be signed by the course instructor.

(C) No course instructor shall certify that a participant has successfully completed such course unless such participant attended the course for the entire six hours required and successfully completed the written exercise and the skills demonstration.
