(a) Fees are as follows:
(1) a request for an initial application for
licensure as an occupational therapist or occupational therapy assistant is ten
dollars ($10.00);
(2) application
for issuance of a license or re-issuance of an expired license is one hundred
dollars ($100.00);
(3) annual
renewal of a license is fifty dollars ($50.00);
(4) late renewal of a license is an
additional fifty dollars ($50.00).
(b) Fees shall be non-refundable and shall be paid in cash or in the form of a cashier's check, certified check or money order made payable to the North Carolina Board of Occupational Therapy. Personal checks shall be accepted for payment of renewal fees.
