(a) The Form BAR-2 shall be filed when there is a change of management at any barber shop. It requires the following:
(1) the name, mailing
address, and permit number of the barber shop before the change of
management;
(2) the name of the
barber shop after the change of management, if the barber shop name
changes;
(3) the name, address, and
certificate number of the new manager or managers;
(4) if the barber shop was closed at the time
the Form BAR-2 was submitted, the date of closure; and
(5) the former manager's name and certificate
number.
(b) The Form BAR-2 shall be notarized.
