In preparation to petition the court under G.S. 108A-106(f), documentation must be made of specific information indicating that:

(1) the adult lacks the capacity to consent;

(2) the adult is in need of protective services;

(3) the adult is being financially exploited; and

(4) no one else is able or willing to arrange for protective services.
