Section 6661.5 - Reports

(a) Each grantee receiving payments hereunder shall submit to the Office, by such time as the Office shall specify and following guidelines prepared by the Office, a report which shall include:

(1) the manner in which grant payments were expended;

(2) a description of the scope, status and quality of the project funded;

(3) the extent to which the combined center reduced expenditures or realized savings;

(4) the impact and effectiveness of this program in meeting the community's needs for senior citizen services, residential health care and child day care and the social benefit the combined center provided to the children and senior citizens in the program;

(5) the extent to which the combined center coordinated services with other community services;

(6) the manner in which all available reimbursement for services has been sought, and the manner in which such reimbursement was expended; and

(7) such other information as the Office shall require.

(b) The Office shall incorporate a summary of the reports required by subdivision (a) of this section and incorporate them into an annual report, including recommendations for legislation, which shall be submitted to the Governor and the Legislature by the conclusion of each State fiscal year for which funds are available.
