Section 113.15 - Verification of appeal papers

(a) All appeal papers other than memoranda of law must be verified by the party in whose name said papers are filed. When an appeal is taken by a community board the appeal papers must be verified by a member of such board who is familiar with the facts underlying the appeal, pursuant to a resolution of the community board.

(b) The affidavit of verification shall be in the following form:
