Section 22.4 - Contents of certificate

(a) The contents of the certificate of literacy shall be as follows:

(1) the full name of the applicant, written in ink by the applicant in the presence of the examiner;

(2) the printed, stamped or written signature of the commissioner;

(3) the date of issuance of the certificate; and

(4) the full name and position of the examiner, written by examiner.

(b) Where the certificate is issued by the superintendent of schools, the certificate shall also contain:

(1) name of city, village or town where certificate is issued; and

(2) printed, stamped or written signature of the superintendent of schools in whose district the certificate of literacy is issued.

(c) Where the certificate is issued by the central registration board, the certificate shall also contain:

(1) name of county where the certificate of literacy is issued; and

(2) written signatures of all members of the central registration board issuing the certificate of literacy.
