Upon reading and filing the petition of ________ duly verified the ________ day of ________, 19 ________, applying for the appointment of a limited guardian of the property of ________, a (mentally retarded) (developmentally disabled) person, and the certifications of ________, dated ________, 19 ________, and ________, 19 ________, and the matter having come on to be heard before this Court on ________, 19 ________, and the Court having found that ________ is a (mentally retarded) (developmentally disabled) person, incapable of handling his/her affairs by reason of (mental retardation) (developmental disability) and that such condition is permanent in nature or likely to continue indefinitely, and that said person is at least eighteen years of age and is wholly or substantially self-supporting by means of his/her wages or earnings from employment, and that the best interests of said person would be promoted by the appointment of a limited guardian of his/her property [and the proposed guardian having filed a bond with sureties pursuant to statute, which has been approved by the Surrogate] it is hereby

ORDERED that ________, upon taking the official oath and filing the designation as required by law, be and hereby is appointed limited guardian of the property of ________, and that letters be issued accordingly, and it is further

ORDERED that these letters are limited to the receiving and administration of only such property of ________ as shall be received from other than his/her wages or earnings, and it is further

[If applicable] ORDERED that the filing of the bond is dispensed with and the guardian is directed to collect and receive the money and property belonging to ________ jointly with ________ and it is further

ORDERED that the guardian file with the Court the annual account required by law.
