ADVISEMENT TO DEFENDANT
YOUR FAILURE TO CONFORM TO ANY OF THE FOLLOWING CONDITIONS OF YOUR PROBATION, INCLUDING THE FAILURE TO REPORT TO YOUR PROBATION OFFICER, IS A VIOLATION OF PROBATION.
