(a) Year
2000 reporting requirements.
(1) Water supply
systems serving the public. The owner(s) of each water supply system serving
the public and subject to requirements under sections
890.50 and
890.51 of this Part, and the
ground water protected area regulations for southeastern Pennsylvania, shall
report the following data for the year 2000 to the designated agency:
(i) Source data.
(a) Withdrawals. The following data shall be
reported for each withdrawal source separately (i.e., each well or surface
water intake):
(1) year 2000;
(2) facility identification code (state
code);
(3) state permit
number;
(4) DRBC docket
number(s);
(5) facility
name;
(6) source type: surface
water/ground water;
(7) source name
and/or source ID number;
(8)
withdrawal location:
(i)
municipality;
(ii)
county;
(iii) latitude and
longitude (method used); and
(9) monthly withdrawal amount (million
gallons [MG]).
(b) Bulk
purchases from other suppliers. The following data shall be reported for each
bulk purchase separately:
(1) bulk supplier
name;
(2) bulk supplier's source
name(s); and
(3) monthly bulk
purchase amount (MG).
(ii) Service area data. The following data
shall be reported separately for each county served:
(a) service area name(s);
(b) service area location(s):
(1) municipalities;
(2) counties;
(c) population served;
(d) total annual water use by category (MG)
(to the extent available, provide a breakdown of water use by the following
categories):
(1) residential metered
(including apartment complexes);
(2) commercial metered;
(3) institutional metered;
(4) industrial metered;
(5) bulk sales;
(6) other metered (specify);
(7) unaccounted for (defined as the amount of
water entering the distribution system minus the amount of water delivered
through service meters);[FN1] and
(8) total;
(e) wastewater disposal (if known);
(1) facility identification code (state
code);
(2) state permit number;
and
(3) facility name;
(f) United States Geological
Survey (USGS) quadrangle map (1:24K) or other equivalent or higher resolution
map identifying withdrawal, service area and discharge locations.
(2) Other withdrawals.
Each person, firm, corporation, or other entity, except water supply systems
serving the public, subject to requirements under section
890.51 of this Part and the ground
water protected area regulations for southeastern Pennsylvania, shall report
the following data for the year 2000 to the designated agency. The data shall
be reported for each withdrawal source separately (i.e., each well or surface
water intake):
(i) year 2000;
(ii) facility identification code (state
code);
(iii) state permit
number;
(iv) DRBC docket
number(s);
(v) facility
name;
(vi) source type: surface
water/ground water;
(vii) source
name and/or source ID number;
(viii) use category:
(a) self-supplied commercial;
(b) self-supplied industrial;
(c) self-supplied institutional;
(d) self-supplied agriculture (including crop
irrigation, nurseries, aquaculture and livestock);
(e) self-supplied nonagricultural irrigation
(i.e., golf course, country club, athletic field);
(f) self-supplied mining;
(g) self-supplied thermoelectric
power;
(h) self-supplied
hydroelectric power;
(i)
self-supplied other (specify) ________;
(ix) withdrawal location:
(a) municipality;
(b) county;
(c) latitude and longitude (method
used);
(x) use location:
(a) municipalities;
(b) county;
(c) latitude and longitude (method used) (if
known);
(xi) acres
irrigated (for irrigated uses only);
(xii) monthly withdrawal amount
(MG);
(xiii) evaporative loss
amount (MG) (if known);
(xiv)
product use amount (MG) (if known);
(xv) is water recycled (Y/N)? if yes, what
percent is recycled? (if known);
(xvi) are withdrawals metered
(Y/N)?;
(xvii) wastewater disposal
(to the extent available, provide the following information on wastewater
disposal):
(a) type (municipal wastewater
treatment plant, package wastewater treatment plant, subsurface discharge
system, other);
(b) facility
identification code (state code);
(c) state permit number;
(d) facility name;
(e) discharge location:
(1) surface water body (wastewater treatment
plants only);
(2)
municipality;
(3) county;
(4) latitude and longitude (method used) (if
known);
(f) estimated
monthly discharge amount (MG); and
(g) is wastewater reclaimed (Y/N)? if yes,
what percent is reclaimed? (if known);

equivalent or higher resolution map identifying withdrawal, use, and discharge
locations.
