Section 4000.2 - Arbitration requests

Arbitration may be initiated by a written request to the State Mediation Board which shall include:

(a) The complete names, addresses and telephone numbers of both parties (and the international affiliation of the labor organization).

(b) The names, addresses and telephone numbers of the attorneys (if any) representing each party.

(c) The text of the contract clause providing for arbitration and the date the contract expires, or a written submission signed by both parties, designating the board as the arbitration agency.

(d) A statement of the specific issue to be arbitrated, including names of the persons to be affected by the arbitrator's award.

(e) A statement or indication that a copy of the request for arbitration has been served on the other side.

(f) The signature and title of the person filing the request for arbitration.
