Section 1.12 - Replacement of lost license, permit or registration card

A licensee, registrant or permittee who has lost his license, registration card or permit, after identifying himself to the satisfaction of the commission, shall be issued a temporary license, registration card or permit, valid for a period of not more than 30 days. He shall also submit a sworn application for replacement of his lost license, registration card or permit and, upon his payment of a replacement charge of $15, the commission shall issue to him a new license, registration card or permit.
