(a) Public welfare agencies shall report, monthly, data other than vendor medical information, on form SC-33 consisting of three sections as follows:
(1) State charges under care during the
month;
(2) payments made during the
month;
(3) removals and
burials.
(b) A report on all dependent Indians living on reservations shall be submitted by the social services districts. The report shall cover a calendar month. Beginning with the month of July 1947, the original of the revised report on form SC-33 shall be submitted to the office by the 15th day of the month following the month for which the report is prepared.
(1) An approved
State charge is a person for whom a written approval has been given by the
office and filed with the social services district.
(2) A presumptive State charge is a person
who, in the opinion of the social services district, has State charge status,
but for whom no written approval has been given by the office.
(3) State charges shall be reported according
to the type of assistance or care received.
