Section 651.4 - Statistical report of medical care, services and supplies provided through medical assistance vendor payments

Social services districts administering AABD, ADC and MA shall report semiannually to the department, Office of Research and Statistics, on vendor medical care, using individual record cards, form RS-108, Annual Record of Vendor Medical Care or an approved local equivalent form. Data for the half-year January through June, shall be submitted on such form by the first day of August next succeeding, and data for the complete year, January through December, shall be submitted on such form by the first day of February next succeeding.
