(a) For purpose
of preparation of annual reports for the New York State Legislature, social
services districts must report monthly to the office information on families
and individuals receiving public assistance benefits, including, but not
limited to, the following:
(1) Applicants
seeking public assistance including but not limited to:
(i) number of applicants accepted to
participate in each program;
(ii)
number of applicants who are denied public assistance and the reason for
denial;
(iii) number of applicants
who were previous recipients of public assistance on or after December 2, 1997
including length of time previously on assistance; reason for initial
termination; length of time between termination and reapplication; and reason
for reapplication;
(iv) number of
applicants who have resided in the State for fewer than 12 months.
(2) Recipients of public
assistance including but not limited to:
(i)
number of terminated recipients of public assistance and reason for
termination; and
(ii) number of
recipients that move between public assistance programs.
(3) Work activities subsidized or otherwise
sponsored by the office or social services districts, including but not limited
to:
(i) number of recipients participating in
work activities;
(ii) number of
recipients receiving an exemption from participating in work
activities;
(iii) the duration of
and reasons for such exemptions;
(iv) number of participants receiving
subsidized child care and the number of exemptions granted due to lack of
available child care;
(v) the type
of work activities;
(vi) hours of
work per week per recipient;
(vii)
duration of enrollment in such work activity;
(viii) reason for termination from work
activity;
(ix) number of requests
for administrative hearings related to work activity participation including
the nature of such requests by general categories and the outcome of such
hearings;
(x) number of
participants receiving subsidized transportation services;
(xi) participant retention in work activities
by type of work activity;
(xii)
length of time before assignment to a work activity;
(xiii) number of recipients changing work
activity assignments and reasons for such changes; and
(ix) expenditures by social services
districts in support of work activities.
(4) Unsubsidized work activities including
but not limited to:
(i) number of recipients
hired;
(ii) initial rate of pay by
either annual salary or wage basis;
(iii) retention rates;
(iv) means of obtaining unsubsidized
employment including, but not limited to, referral by the social services
district and direct contact with employer; and
(v) number of participants receiving
subsidized child care and/or transportation services.
(5) Recipient training and education
activities including but not limited to:
(i)
number of recipients in training or educational activities by type of
activity;
(ii) number of recipients
mandated to attend training or education activities that fulfill work
participation requirements;
(iii)
length and duration of training or education activities including the number of
hours per week and duration;
(iv)
number of recipients receiving subsidized child care and/or transportation
services;
(v) number of recipients
transferred to work activities;
(vi) to the extent available, the number of
recipients who obtain work directly from training or education activities
including starting salary and employee retention rates; and
(vii) expenditures by social services
districts in support of training and education activities.
(6) Recipient sanctions including, but not
limited to:
(i) number of sanctions, by type
of sanction;
(ii) number of
sanctions with defined minimum period, by type of sanction;
(iii) average length of sanction until
recipient compliance, by type of sanction;
(iv) amount of sanction, by type of
sanction;
(v) period of time
starting on or after December 2, 1996 that the participant was in the program
prior to the sanction;
(vi) number
and outcome of administrative hearings related to sanctions, by type of
sanction; and
(vii) number of
sanctions against entire assistance unit.
(7) Case closings, including but not limited
to:
(i) number of case closings;
(ii) reason for case closing including, but
not limited to, self-termination without reason;
(iii) length of time receiving assistance on
or after December 2, 1996 before case closing;
(iv) on an aggregate basis, the length of
time recipients do not receive public assistance in New York State following
case closing occurring on or after December 2, 1996; and
(v) number of former public assistance
recipients receiving subsidized child care and/or transitional medical
assistance.
(8)
Substance abuse treatment programs including but not limited to:
(i) number of recipients receiving substance
abuse treatment services for which the cost of such services is paid for by the
social services district using public assistance monies;
(ii) length of reimbursed recipient
participation in treatment program; and
(iii) types of substance abuse treatment
programs receiving reimbursement from social services districts.
(9) Pregnancy prevention programs
funded through Federal monies received by the State pursuant to title IV-A of
the Social Security Act including, but not limited to:
(i) number of program participants;
(ii) duration of program participation;
and
(iii) types of services
provided.
(10) Disabled
and work-limited recipients of public assistance pursuant to provisions of
chapter 436 of the Laws of 1997 (the Welfare Reform Act) including, but not
limited to:
(i) number of persons applying
for public assistance that are screened;
(ii) number of current recipients of public
assistance screened;
(iii) number
of persons applying for public assistance that are found to be
disabled;
(iv) number of current
recipients of public assistance found to be disabled;
(v) number of persons applying for public
assistance found to be work-limited;
(vi) number of current recipients of public
assistance found to be work-limited;
(vii) duration of finding by
category;
(viii) number of changes
in work activity exemption status;
(ix) number and outcome of administrative
hearings related to the disability screening and the determination of work
activity exemptions; and
(x) data
relating to work and training and education activities pursuant to paragraphs
(3), (4) and (5) of this subdivision.
(11) Domestic violence screening process
including but not limited to:
(i) number of
applicants and recipients screened for evidence of domestic violence;
(ii) number of positive findings;
(iii) number of waivers by category granted
as a result of a positive finding; and
(iv) duration of such waivers.
(b) The information
required by this section shall be collected using the Welfare Management
System, the Welfare to Work Caseload Management System, or other automated
systems or forms as determined by the commissioner of the office . Social
services districts shall assist the office in collecting the information in
accordance with instructions from the office.
(c)
(1)
Scientifically acceptable statistical sampling methods may be used to collect
and maintain data when authorized by the Federal Department of Health and Human
Services or when the commissioner of the office determines that collection of
such data by any other means is not practicable or would result in significant
additional costs.
(2) Unless
otherwise required by the Federal Department of Health and Human Services, the
sample size shall be limited to the size required under Federal statute for
SNAP quality control purposes.
